Your First Visit
On your first visit, information will be gathered and an assessment will be conducted to assist in the development of your treatment plan. Your goals for therapy will be discussed.
Please bring your insurance card(s) and a list of your current medications. If you have a deductible and/or copay through your insurance plan, please bring this information with you. Also, please call your insurance company to find out if preauthorization is required for use of your behavioral health benefits.
If the client is a child or adolescent, please do not bring your child or adolescent to the first appointment, as required consent forms and background information must be gathered before treatment of minors can begin.
If you are a court-appointed guardian or share custody of a child client, please bring a copy of your final custody agreement or the court order confirming you as guardian.
